Moore has spent a period on loan at Derby this season and the Evening Post report that by this time next week he could be a Swansea player after he held talks with Brendan Rodgers.
The Post also reports that both Derby and Middlesbrough are keen on a January move for Moore with Swansea reportedly confident that they have won the place to sign the player.
Moore has struggled at the Hawthorns since completing a move from Aston Villa in the summer of 2008 and was not included in their 25 man squad at the start of the season and has scored just five goals for the club in his time there.
No quotes back up the Evening Post article but they seem confident that a move could be imminent although weekend rumours suggested that Eidur Gudjohnsen and Tamas Priskin could also be in the frame.
