Health workers are too important to the country to self isolate. They're told to carry on until they actually have symptoms.
Writing this it feels like I've written similarly earlier in the year.

I get the non self isolation but surely they have to be tested and self isolate until a negative result is received?

They'll be tested only if a care home that they go to has covid cases.
Tragically it could be them that took it there in the first place.
