I have never understood why a player who is fouled and injured, thus needing a physio on the pitch, has to go off to the touch line and wait for play to resume before being allowed back on. This is no advantage at all to the team whose player was fouled.

If a decision (throw, offside, foul, etc) goes against a player it should be a bookable offence to continue to hold onto the ball, wasting like 5-10 seconds if you are winning or for teammates to get back into a position

The rule is there to stop players slowing down a game and/or the opposition by feigning injury. They do not come back on until given permission by the ref.
When I was reffing at local level in Hereford and a player pulled that stunt on me, I'd leave him stewing on the touchline for 30 seconds or a minute to give him the message that he couldn't extract the urine from me without paying a price.
